Enter your current and previous addresses

Before you begin

Gather the following materials:

• Your current and previous addresses for the past two years
• Dates you began and stopped living at those addresses
• If you rent, the landlord contact information for each rental unit
• If you own a home, the mortgage information

Enter your housing history

1. Enter your full current address.

2. Indicate whether you are renting, own a home, or have another housing status, such as if you're living with family - instructions for each are below.
3. Enter the date you began living at your current address.
4. If the date is within the past 2 years, you will need to add any previous addresses you lived since that time, as well as the dates you lived there.

If you are renting

1. Enter the contact information for the landlord. All fields are required.
2. Enter the total rent (if there are multiple people, not just your portion).
3. Enter the share of the total rent you pay.
4. You must verify your rental or mortgage payments, if you have any. You can do so via linking a bank account (recommended), or by uploading documents.
   If you do not make payments for your housing, you can skip this step.

If you own a home

First, indicate whether or not you have a mortgage.

• If Yes, enter the name of your mortgage lender and the amount of your monthly mortgage payment on the following pages.
• If No, provide details about the home you own (for example, when you stopped paying mortgage payments).

If Other

1. Select your housing status from the drop down list. Your options are:
   • With friends or family
   • Student housing/dorm
   • Military housing
   • Other
2. Provide details in the field provided.
